Loyalty Dental:
- Impacted states: AR, AZ, DC, DE, IA, ID, KY, NC, ND, NJ, SC, TX, UT, VA, WI and WY
- No longer require Enrollment Fee
FeeComplete Dental:
- Impacted states: AL, AZ, CA, CO, DE, FL, GA, IL, IN, KS, KY, LA, MN, MO, NC, OH, PA, TN, TX, UT and WI
- No longer require Enrollment Fee
Dental Value DHMO C550:
- Impacted states: IL
Humana Vision:
- Impacted states: NC
Preventive Plus and Veteran Preventive Plus (Not offered via MAPA):
- Impacted states: AR, AZ, CO, TX, VA and WI
- AR and VA will no longer require Enrollment Fee
