New Suitability Form & Questionnaire required February 1, 2015
As previously announced, we have updated our Suitability Form and Questionnaire in all
annuity application packs.
Effective 02/01/2015, the new Suitability Form & Statement of Understanding will be REQUIRED for use in ALL states for Sentinel Plan Annuities. All downloadable application packs on the Agent Portal have been updated with the new forms and paper versions are now available for order. Please be aware that if you previously ordered materials directly from the site, you may have an old form. Please discard if not used before 02/01/2015.
