NEW CS Tactical Multi Asset Index Account Now Available in California!

California’s recent approval of our newest index means you can now use a
single application in California for any of Phoenix’s four core fixed
indexed annuities:

• Personal Income Annuity
• Personal Retirement Choice
• Personal Protection Choice
• Index Select Gold Bonus

In addition, there is a new Allocation/Rider Election form for each
product that includes the new 1-Year and 2-Year CS Tactical Multi Asset
Index (CSTMAI) accounts.

What do you need to do?

• Begin utilizing the new application and Allocation/Rider Election Form, now available on PhoenixSalesNet.com and in fulfilled forms kits
• For clients not allocating to the CSTMAI accounts, old applications will continue to be accepted until
