Information Now Available
The Minnesota Department of Health has created state
specific HIPAA Authorization forms to help expedite
the gathering of medical records, including
psychotherapy and/or chemical dependency records. All
health care providers in Minnesota must accept these
authorization forms. We ask that you do not modify the
forms as this may result in underwriting delays.
