Applications

As you probably know, LifeSecure offers streamlined LTCi
e-applications with expedited underwriting and fast e-signature options. However, if you prefer to complete paper applications with your clients and enter the information later in your Agent Portal, please make sure you are using the correct
applications – both by product and by state:

  • In Florida, Hawaii, and California ONLY, use a state-specific LTC I application.
  • In states where our LTC II product is available, use
    the LTC II application that coincides with the state
    where your client is physically located when signing the application.


Visit your Agent Portal today to find the most up-to-date
versions of our applications and other important documents in our application and form packets, which are available in our PDF Library, by state.
