Important: Advantage Plus Durable Medical Equipment Rider Update

Effective January 2, 2014, the

Durable Medical Equipment Rider will be discontinued on our Advantage Plus product. We have revised the brochures, applications, rates and outlines of
coverage to reflect this change.    

Please be advised, effective January 2, 2014 we will no longer be offering the old Ambulance and Skilled Nursing Facility riders in the states that have approved the new riders. All applications dated December 31st and earlier must be submitted by January 6, 2014.

Below is a listing of the states:

AL, AR, DE, GA, HI, ID, IL, IN, IA, ME, MI, MS, MO, MT,
NE, NC, NM, OK, SD, TN, TX, WI and WV.
