Effective September 23, 2013,
all applicants who complete a health application/enrollment form
must be provided with a revised HIPAA “Notice of Privacy Practices”
at the time they apply for coverage.
The Notice must be left with an applicant for the following
products: Advantage Plus hospital indemnity; CHS Protection Plus
lump sum cancer; Critical Care; Recover Cash short-term nursing home
and Supplemental Solutions Schedule of Benefit/Lump Sum Cancer. It
does not need to be left for our 24 Hour Accident or Critical
Provider products.
This revised Notice, while providing all of the required protections
and disclosures as our prior Notice, now includes:
• An individual’s right to be notified of a breach of
unsecured protected health information; and
• A disclosure that the Company is prohibited from using or
disclosing an individual’s genetic information for underwriting
purposes.
