Updated Florida Suitability Questionnaire Now Available

An updated Florida Annuity Suitability Questionnaire (D2624213FL) is now available for your IRA and non-qualified sales. The updated form includes five new sections,
labeled A through and E, to help ensure your clients are making a suitable purchase. Please note pages one through three of the form must be completed, even if clients select an opt-out option on page six.

We will continue to accept the previous version through October 25, 2013. If the previous version is submitted after this date, paperwork will be considered Not in
Good Order (NIGO) and could result in processing delays.

New Client Financial Disclosure Form for Irrevocable Trusts

A new Client Financial Disclosure Information form (D2658913NW) is now available for irrevocable trusts. The new form should be used in place of the current CFD form
(D2624213NW) for all sales where an irrevocable trust is named the owner,
including exchanges, transfers and rollovers.

We will continue to accept the current CFD form for irrevocable trust cases through October 25, 2013. If the new CFD form is not submitted after this date, paperwork will be considered NIGO and could result in processing delays.
