Florida Annuity
Suitability Questionnaire and Disclosure and Comparison Form Required for
All Ages

The Florida Annuity
Suitability Questionnaire and Disclosure and Comparison of Annuity Contracts
form are now required for all issue ages. The forms were previously only
required for clients ages 65 and over. Please note the suitability
questionnaire is only required for IRA and non-qualified sales, and the
disclosure and comparison form is only required for replacement
transactions.

To help ease the new requirement�s
effect on your sales, we will continue to accept new business paperwork for
these cases without the forms through November 15, 2013. 

Updated IL Replacement Form Now Available

An updated replacement form is required for sales in Illinois. The updated
form replaces the previous version and is now available in our


online new business kitting.

We will continue to accept the previous version through November 8, 2013.
After this date, if the previous version is submitted, paperwork will be
considered not in good order and could result in processing delays.

Year-End Processing Deadline

December 13, 2013

To ensure that
applications for loans, distributions, required minimum distribution (RMD)
requests or annuitizations are processed in the 2013 calendar year, all
necessary paperwork must be received in the Home Office and in good order by
December 13, 2013.
