Changes to Client Financial Disclosure Forms for Irrevocable Trusts

Updated Nationwide Version and New California State-Specific Version

A new nationwide Client Financial Disclosure Information form is now available (D2658914NW) for irrevocable trusts. The new form should be used in place of the current form (D2658913NW) for all sales where an irrevocable trust is named the owner, including exchanges, transfers and rollovers. The updated version includes
two new sections with questions pertaining to the purpose of the trust and
representation of the trustee(s).

Additionally, a new California Client Financial Disclosure Information form
is now available (D2658914CA) for irrevocable trusts. This new
state-specific version should be used in place of the current nationwide
version for your California sales where an irrevocable trust is named the
owner.

We will continue to accept the previous versions of the forms through
Friday, February 28. After this date, if previous versions are
submitted, paperwork will be considered not in good order and could result
in processing delays.
